EXTRAGENITAL MALIGNANT MIXED MULLERIAN TUMOR INITIALLY APPROACHED AS COLON ADENOCARCINOMA: CASE REPORT AND LITERATURE REVIEW

Introdução

Malignant Mixed Mullerian Tumors (MMMT), or carcinosarcomas, are rare biphasic neoplasms with an unfavorable prognosis, most often located in the uterus. Extragenital TMMMs are exceptionally rare and their origin is unknown.

Female patient, 60 years old, admitted to an oncologic hospital after left colon resection.

Resultados

Female patient, 60 years old, admitted to an oncologic hospital after left colon resection. The clinical picture included abdominal pain and changes in bowel habits, as well as the presence of a pelvic mass on physical examination. A CT scan demonstrated an expansive hypodense lobulated lesion, with heterogeneous enhancement, in the left iliac fossa, with no sigmoid cleavage plane, measuring 59x72x100 mm. A diagnostic misinterpretation led to an initial surgical approach to what was considered to be a colonic adenocarcinoma. The pathology report revealed a Mullerian carcinosarcoma, and the immunohistochemical panel confirmed a biphasic malignant neoplasm with a sarcomatous component (60% of the sample) associated with epithelial high-grade serous carcinoma (40% of the sample). The patient underwent a total hysterectomy with BSO and a complete resection of the residual peritoneal lesion. The residual peritoneal lesion was MMMT, but it was not observed in the uterus, tubes, or ovaries, configuring an extragenital MMMT case. After surgery, the patient received adjuvant therapy with chemotherapy (carboplatin and paclitaxel) and radiation therapy.

Conclusões

The present report describes an extremely rare case of Extragenital MMMT with an unknown primary site. Due to the rarity of the disease and the limited data on its management, treatment is based only on the particularities of each case.
